About the Role:

We are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our clients in their homes. As a Home Health Occupational Therapist, you will work closely with our multidisciplinary team to develop and implement individualized treatment plans that promote independence, safety, and overall well-being.

Responsibilities:
  • Conduct thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
  • Develop and implement individualized intervention plans to achieve favorable outcomes.
  • Evaluate home environments for hazards or barriers to more independent living.
  • Identify equipment needs and environmental adaptations to enhance outcomes for clients.
  • Continually assess and revise the occupational therapy care plan and participate with nursing in the multidisciplinary care plan.
  • Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities necessary to promote health, safety, and independent living.
Requirements:
  • Licensed as an Occupational Therapist in AZ.
  • Must be willing to travel in assigned territory.
  • 1 year of experience as an Occupational Therapist preferred.
