Join Our Team as a Home Health Registered Nurse

Alana Healthcare/Preferred Home Health Providers is seeking a skilled and compassionate Home Health Registered Nurse to join our team in Leesburg and other locations. As a Home Health Nurse with Alana, you will have the opportunity to build meaningful relationships with patients and provide high-quality care in the comfort of their own homes.

Key Responsibilities:
  • Deliver high-quality patient care in accordance with applicable laws, regulations, and policies
  • Focus on the medical needs and treatment of patients, adhering to an active plan of care
  • Assess and chart observations of the patient's condition at each visit
  • Conduct patient evaluation tasks, including reviewing medication and vital signs
  • Administer prescribed medication, treatments, and therapies
  • Coordinate care with physicians, PT, and other individuals in the patient's care plan
  • Provide education to patients, caregivers, and family members
Requirements:
  • Active RN/LPN License to practice in the State of Florida
  • One year of clinical-based experience with a specialty in home health preferred
  • Current CPR certification
  • Self-driven and accountable to provide excellent patient care and attention to detail
  • Ability to deliver skilled services, including assessment and skilled observation, wound care, and dressing changes
  • Maintain a level of knowledge to navigate information through an electronic medical records system
  • As a condition of employment, must successfully complete a drug test and background check, including a criminal history check and character references
