About the RoleThe Physical Therapist will be responsible for providing high-quality physical therapy services to patients in their homes. This will involve conducting comprehensive assessments, developing and implementing treatment plans, and collaborating with other healthcare professionals to ensure the best possible outcomes for our patients.
Responsibilities- Conduct initial and ongoing assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Develop and implement treatment plans in consultation with physicians and revise as necessary.
- Assist in the development and implementation of interdisciplinary client care plans as it pertains to physical therapy.
- Teach, supervise, and counsel patients and their families in physical therapy programs.
- Supervise Physical Therapist Assistants at least every 30 days.
- Communicate with physicians and other healthcare professionals regarding patient status and obtain additional orders as necessary.
- Write clinical/progress notes on the date of the visit and incorporate same in the client's record according to company policy.
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- At least one year of experience as a Physical Therapist.
- Home healthcare experience is preferred but not essential.
- Electronic medical record experience is preferred but not essential.
