Home Health Physical Therapist Assistant Job Description

At Interim HealthCare, we are seeking a skilled Physical Therapist Assistant to join our home health team. As a Home Health Physical Therapist Assistant, you will play a vital role in providing personalized physical therapy to patients in their homes.

Key Responsibilities:

  • Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury.
  • Work as part of a home health team, which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient's plan of care and goals.
  • Act at the direction and under the supervision of the treating Physical Therapist and in accordance with the plan of care.
  • Assist the treating Physical Therapist in documenting exercises in patient's chart and printing out exercises for home as directed by treating Physical Therapist.
  • Participate in educating the patient and the family with exercises under the direct supervision of the treating Physical Therapist, and other caregivers.

Requirements:

  • Graduate of an accredited Physical Therapist Assistant Program and active PTA license in Colorado.
  • Minimum of one (1) year of physical therapy assistant experience preferably in a home healthcare setting.
  • CPR Certification (demonstration course required).
  • Pass a background check and drug test (we are required to test for THC).
  • Knowledge of state and federal home health regulations.
  • Good clinical judgement, strong interpersonal skills, dexterity, and compassion.
