About the Role

The Registered Nurse provides skilled professional nursing care to patients, families, and caregivers as prescribed by physicians and in compliance with state regulations, patient needs, and Bluegrass Care Navigators policies.

Responsibilities
  • Delivers high-quality patient care, adhering to established standards and principles.
  • Collaborates with interdisciplinary teams to develop and implement patient care plans.
  • Provides education and support to patients, families, and caregivers on disease management, treatment options, and end-of-life care.
  • Conducts thorough assessments, identifies patient needs, and develops individualized care plans.
  • Administers medications, performs procedures, and provides wound care as needed.
  • Communicates effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participates in quality improvement initiatives, contributing to the enhancement of patient care and outcomes.
Requirements
  • Graduate of an accredited school of professional nursing.
  • Current RN licensure through the State Board of Nursing.
  • At least one year of general nursing experience, preferably in hospice, home health, medical, surgical, or critical care settings.
  • Excellent observation, nursing judgment, and communication skills.
  • Ability to provide good written documentation in a timely manner.
  • Self-directed with the ability to work with minimal supervision.
  • Flexibility and cooperation in fulfilling role obligations and ability to work within an interdisciplinary team.
