Nurse Practitioner Job Description

Zufall Health Center is seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a key member of our clinical staff, you will provide high-quality patient care and work collaboratively with our medical team to deliver exceptional healthcare services.

Key Responsibilities:
  • Provide direct clinical care to patients, including diagnostic and therapeutic assessments, treatment of conditions and illnesses, and referral and follow-up of patients.
  • Work closely with collaborating physicians and follow established guidelines for Nurse Practitioner and physician collaboration.
  • Lead assigned clinical support staff to improve patient care and outcomes.
  • Participate in night and weekend call coverage, consulting with collaborating physicians or the Chief Medical Officer as needed.
  • Assist patients in accessing community resources, referrals, and other services as clinically indicated.
  • Remain mindful of and achieve Federal and health center productivity standards.
  • Participate in clinical performance improvement activities and conduct peer reviews as needed.
  • Ensure compliance with local, state, and federal rules and regulations, and maintain current licensure, credentialing, and educational activities.
  • Participate in in-service educational programs and continuing education, and teach and train other health center staff and NP students as needed.
  • Participate in community health initiatives, including health fairs, special immunization sessions, and other community and outreach activities.
  • Maintain membership in appropriate professional associations and adhere to the Code of Conduct, policies, and procedures of Zufall Health Center.
Requirements:
  • Master's degree or Doctorate from an Advanced Nurse Practitioner training program from an accredited college or university.
  • Certification as an NP in the area of training.
  • Possession of an RN license and an Advance Nurse Practitioner license from New Jersey.
  • Current certification as an Advance Nurse Practitioner in Family Practice.
  • Possession of a valid DEA and CDS registration, and BLS Certification.
  • Minimum of one year experience in a community health setting preferred.
  • Bicultural and bilingual experience valued.
