Key Responsibilities
  • Conduct initial and ongoing supervisory field visits to patients' homes to assess their needs and provide training support.
  • Document patient visits and care plans in the electronic medical record (EMR) in a timely and accurate manner.
  • Orient and supervise Home Health Aides (HHAs) in the field to ensure they are equipped to provide high-quality care.
  • Collaborate with the clinical care team to convey significant changes in patient condition, emergency interventions, or care plan changes.
  • Ensure compliance with regulatory requirements and VNS Health policies and procedures.
Requirements
  • Current license to practice as a Registered Nurse in New York State.
  • Minimum of two years of experience as an RN in a home care setting.
  • Valid driver's license may be required.
