Job Overview

POSITION SUMMARY

The Clinical Pharmacist plays a vital role in the management and coordination of pharmacy services, including compounding, inpatient/outpatient activities, dispensing medications, and conducting clinical monitoring.

EDUCATIONAL REQUIREMENTS

  1. Graduation from an accredited School of Pharmacy.
  2. Current licensure and registration as a pharmacist in good standing.
  3. Completion of an aseptic technique course recognized by the American Society of Hospital Pharmacists (ASHP) or an equivalent program.

EXPERIENCE REQUIREMENTS

  1. 1 to 2 years of experience in a hospital or specialty pharmacy setting.
  2. Strong oral and written communication abilities.
  3. Exceptional interpersonal skills.
  4. New graduates are encouraged to apply.

EMPLOYMENT TYPE

  1. Full-time position, requiring 40 hours per week, Monday through Friday.
  2. Work schedule will be determined by the supervisor.

KEY RESPONSIBILITIES

Clinical Responsibilities
  1. Maintain up-to-date knowledge and skills in pharmaceutical care services relevant to assigned duties.
  2. Engage in decision-making regarding pharmaceutical care services, including:
    1. Assessing patient pharmaceutical needs.
    2. Planning the provision of pharmaceutical services.
    3. Contributing to the overall care plan.
    4. Participating in the integrated care plan.
  3. Review physician prescription orders for appropriateness regarding dosage, drug selection, potential interactions, administration routes, and stability.
  4. Ensure comprehensive patient database documentation, including height, weight, medical history, lab results, and current medications.
  5. Assist in maintaining compliance with state and federal regulations, including documentation for the Joint Commission and FDA mandates.
  6. Coordinate necessary laboratory tests and discuss results with healthcare team members.
  7. Document all communications with healthcare professionals and patients in the patient record.
  8. Uphold safety standards for patients and staff through diligent pharmacy practices.
  9. Provide drug therapy education to patients.
  10. Participate in interdisciplinary patient care meetings.
  11. Ensure compliance with HIPAA regulations and maintain confidentiality in all processes.
Operational Responsibilities
  1. Support the pharmacist-in-charge in the implementation of new procedures.
  2. Provide clinical and technical assistance to physicians, nurses, and patients.
  3. Assist in training and supervising pharmacy technicians, interns, and other pharmacy staff.
  4. Oversee the compounding and dispensing of sterile products.
  5. Deliver in-service training to nursing staff on drug therapy aspects.
  6. Help manage inventory and order medications and supplies.
  7. Assist in tracking equipment and performing preventative maintenance.
  8. Maintain daily logs for pharmacy operations.
  9. Participate in the on-call system for patient care support.
  10. Integrate new patient engagement and clinical technologies into daily workflows.
  11. Perform additional tasks as assigned by the pharmacy director.
  12. Remote pharmacists may occasionally be required to work on-site.
Quality Improvement Responsibilities
  1. Engage in the company's quality assurance initiatives and recommend policy changes based on data evaluations.
  2. Report any unusual occurrences through the risk management process.
