Job Summary

The University of Texas Health Science Center at Tyler seeks a highly skilled Licensed Professional Counselor (LPC) to join our team at the Lake Street Clinic. As a key member of our interdisciplinary team, you will provide essential services to chronically mentally ill patients, helping them achieve their treatment goals and transition to community-based care.

Key Responsibilities
  • Manage a caseload of chronically mentally ill patients, conducting ongoing assessments and developing individualized treatment plans.
  • Collaborate with the patient's recovery team to complete competency reports for court, as needed.
  • Provide interdisciplinary treatment planning, discharge planning, and follow-up services for assigned patients.
  • Lead group psychotherapy and case management sessions, ensuring a minimum of three times per week.
  • Maintain confidentiality of patient records and adhere to strict confidentiality protocols.
  • Design and implement psychological activities and programs to support patient recovery.
  • Administer and interpret psychological tests, conduct assessments, and diagnose patients as needed.
  • Conduct behavior analyses, design behavior modification and skill acquisition plans, and monitor patient progress.
  • Facilitate and document weekly individual interactions and interventions to aid patients in understanding their psychiatric treatment needs.
  • Review psychological reports for completeness and accuracy, ensuring high-quality documentation.
  • Provide individual or group counseling services for all age ranges, addressing a range of mental health concerns.
  • Fulfill clinical documentation requirements to accurately evaluate patient progress, identify ongoing educational needs, and ensure seamless discharge and aftercare planning.
  • Establish and maintain family contracts, ensuring continuity of care and providing family education as needed.
  • Attend and participate in scheduled treatment team meetings, contributing to patient treatment planning and progress.
  • Act as a mentor to less experienced LPCs, promoting high-quality patient care and services.
  • Serve as primary case manager for patients, coordinating interdisciplinary assessments and developing psychosocial summaries within 3 days of admission.
  • Assist with coordination of admissions, discharges, and furloughs, ensuring seamless patient transitions.
  • May supervise the work of others, providing guidance and support as needed.
Requirements
  • Master's degree in Counseling, Social Work, or Psychology.
  • Current State of Texas licensure with scope of practice encompassing group, individual, and family therapy.
  • Three years of healthcare experience, with a focus on mental health services.
  • Bilingual skills a plus.
  • Current AHA Heartsaver CPR certification.
  • Physical ability to perform SAMA.
