Case Manager I/II

Job Summary:
The Case Manager I/II position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The successful candidate will obtain in-depth social histories, develop and assure community and/or agency resources to meet the consumer's needs, and serve as a Qualified Intellectual Disabilities Professional (QIDP) providing monthly monitoring of the program.
Responsibilities:
- Provide case management services to individuals with intellectual and/or developmental disabilities
- Obtain in-depth social histories and develop community and/or agency resources to meet consumer needs
- Serve as QIDP and provide monthly monitoring of the program
- Meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines
Requirements:
- Bachelor's degree in a human service field or a related field
- One year of general experience working directly with individuals with developmental and/or intellectual disabilities or one year of professional experience working directly with individuals with developmental and/or intellectual disabilities in a related field
- Valid permanent Oklahoma Nursing license (for Level II only)
Working Conditions:
This position requires extensive travel and must possess a valid driver's license and maintain required car insurance.
