Job Summary

Rise Life Services, An Aid to The Developmentally Disabled, is seeking a skilled Licensed Practical Nurse to join our team in a residential setting. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to individuals with intellectual disabilities and mental health challenges.

Key Responsibilities
  • Provide nursing care and support to individuals in a residential setting, including administering medications, monitoring vital signs, and assisting with daily living activities.
  • Collaborate with the Director of Nursing, Registered Nurse, and residential staff to ensure the delivery of high-quality integrated health and continuity of care.
  • Assist with delegated nursing functions, including data collection, lab results, and diagnostic testing.
  • Communicate effectively with staff, physicians, and Registered Nurses to ensure seamless care coordination.
  • Participate in Inter-Disciplinary Team Meetings, division meetings, and staff meetings to ensure alignment with agency mission and vision.
Requirements
  • Current LPN licensure in the state of practice.
  • Ability to work in partnership with all personnel within the organization to ensure high-quality clinical services.
  • Strong communication and interpersonal skills.
  • Ability to manage multiple priorities and be flexible to meet the needs of a diverse population.
