Job Summary

Rise Life Services is seeking a skilled Licensed Practical Nurse to join our team as a Residential Care Specialist. In this role, you will provide high-quality nursing care to individuals with intellectual disabilities in a residential setting.

As a Residential Care Specialist, you will work closely with our interdisciplinary team to develop and implement individualized care plans that promote the health, well-being, and independence of our residents.

Key Responsibilities:

  • Provide nursing care and support to residents in a residential setting
  • Assist with data collection and documentation of resident care
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
  • Provide education and training to staff on nursing procedures and best practices
  • Participate in Interdisciplinary Team Meetings and contribute to the development of care plans

Requirements:

  • Valid NYS LPN license
  • Valid NYS driver's license
  • Experience working with individuals with intellectual disabilities preferred
  • Knowledge of OPWDD state and federal regulations preferred
