The Northeast Georgia Health System, Inc. is seeking a highly skilled Social Worker to join our Ambulatory Care team. As a key member of our Care Management team, you will play a vital role in providing social work services to patients receiving care in our primary care setting.
Key Responsibilities- Assess the social, psychological, cultural, environmental, and financial factors unique to each patient and develop a plan to address non-medical factors impacting their health.
- Collaborate with Primary Care Physicians and other healthcare team members to ensure effective care coordination.
- Maintain current knowledge of community resources and facilitate connections between patients and resources as needed.
- Provide education and support to patients and caregivers on self-management of chronic conditions, lifestyle changes, and behavior modifications.
- Document assessment results, action items, and patient progress in the Electronic Medical Record (EMR).
- Prepare reports as required by federal, state, and local programs.
- Participate in Quality Improvement (QI), Quality Assurance (QA), and Continuous Improvement Activities (CQI) as needed.
- Masters Degree in Social Work from an accredited School of Social Work.
- One to three years of experience in a hospital, agency, or institution providing related healthcare services.
- Excellent written and verbal communication skills, with the ability to work effectively in both independent and team settings.
- Current knowledge of state and federal programs providing medical care and financial support to individuals.
- Passion for working with individuals from diverse cultural and socio-economic backgrounds.
