BAYADA Home Health Care is seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a Home Health Visits professional. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

About the Role

We are looking for a skilled and experienced RN to provide high-quality, patient-centered care to our clients in the comfort of their own homes. As a Home Health Visits RN, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing ongoing support and education to patients and their families.

Responsibilities
  • Assess patient needs and develop individualized care plans
  • Provide high-quality, patient-centered care to clients in their homes
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and outcomes
  • Participate in ongoing education and training to stay up-to-date on best practices
Requirements

To be successful in this role, you will need:

  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience is strongly preferred, but not required
  • Graduation from an accredited and approved nursing program
  • Solid computer skills and prior experience with electronic medical records (EMR) is preferred
