About the RoleWe are seeking a compassionate and skilled Private Duty Nurse RN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, working closely with our patients, families, and healthcare teams to ensure seamless care transitions.
Responsibilities- Implement and manage patient care plans in collaboration with physicians and healthcare teams
- Conduct thorough patient assessments and coordinate care to meet individual needs
- Administer medications, treatments, and therapies as prescribed
- Document patient care and progress in our electronic system
- Provide education and support to patients and families on health promotion and disease management
- Collaborate with Case Managers to ensure timely and effective care delivery
- Active, unencumbered RN license in the state of practice
- Current CPR certification from American Red Cross or American Heart Association
- TB skin test (current within last 12 months)
- 6 months prior hands-on nursing experience preferred
- Reliable transportation
