Responsibilities

The Registered Nurse provides intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care. Key responsibilities include:

  • Assessing and evaluating patient needs and problems
  • Identifying mutually agreed-upon goals for care plans with patients
  • Reporting patient status, updates, and discharge planning process to clinical leadership, attending physician, and hospice physician
  • Performing annual on-site visits to observe and assess each aide
  • Ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and providing Age-Friendly Care

Qualifications

To be successful in this role, you must have:

  • A current unencumbered State Professional Nurse License
  • A valid driver's license and good driving record
  • The ability to work in a field setting and make sound nursing judgment
  • Experience with Microsoft Office
