POSITION OVERVIEW:

KEY RESPONSIBILITIES:

  • Assist clients in navigating the healthcare landscape through education, coaching, and support.
  • Coordinate both behavioral and primary care needs, devising strategies to eliminate obstacles to wellness.
  • Employ behavioral management techniques, crisis intervention, active listening, and motivational interviewing to help clients enhance their life management skills.
  • Develop and implement personalized service plans tailored to individual needs.
  • Exhibit knowledge and adherence to ethical standards through practice with clients and community partners.
  • Participate in weekly team meetings and complete necessary assessments.
QUALIFICATIONS:

Education:
Bachelor's degree in psychology, social work, or a related field.

Experience:
  • Familiarity with Electronic Health Records is preferred but not mandatory.
Licensure:
Must possess a valid driver's license in good standing.

SALARY RANGE:
$19.10 - $26.80
