Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an LPN with Adoration Home Health and Hospice, you will have the opportunity to work in a fast-paced, dynamic environment, providing one-on-one care to patients in their homes.
Responsibilities- Perform a range of nursing procedures within the scope of LPN practice, including administering medications, monitoring vital signs, and providing patient and family teaching.
- Monitor and record patient intake and output, and report any significant changes to the healthcare team.
- Observe, assess, and record patient physical and mental condition, and report any changes to the healthcare team.
- Administer medications as instructed and as ordered by the physician, and document patient files accordingly.
- Attend and participate in multi-disciplinary patient case conferences, staff meetings, and in-service education programs as assigned.
- Cooperate with on-call schedule, weekends, and after-hour visits as necessary.
- Licensed Practical Nurse (LPN) license in good standing.
- Previous experience in home health, hospice, hospital, or skilled nursing preferred.
- Familiarity with electronic health records.
- Strong problem-solving and critical thinking skills.
- Ability to thrive in a fast-paced environment.
- Reliable transportation, valid driver's license, and current automobile insurance.
