Job Summary:

Della Infotech is seeking a highly skilled Pharmacist to join our team. As a key member of our healthcare team, the successful candidate will be responsible for providing safe and high-quality pharmaceutical care to patients across the healthcare continuum.

Key Responsibilities:

  • Provide day-to-day oversight of assigned patients and ensure the provision of safe and effective pharmaceutical care.
  • Work independently in various pharmacist positions within the hospital, including main pharmacy distribution, Med/Surg, step down, ED, ICU, and Antimicrobial Stewardship.
  • Collaborate with healthcare teams to ensure seamless patient care and optimal treatment outcomes.
  • Develop and implement effective medication management strategies to improve patient outcomes.
  • Stay up-to-date with the latest pharmaceutical trends and best practices to ensure the highest level of care.

Requirements:

  • Current State of Pennsylvania Pharmacy license in good standing.
  • Authorization to Administer Injectables required within 120 days of hire.
  • PharmD degree from an accredited institution.
  • Two or more years of health-system pharmacy experience (or equivalent residency training) preferred.
  • One year of acute care (hospital) experience is required for this per diem role.
  • Preferred: Experience with Pyxis MedStation, EPIC.
