Job SummaryThe Ambulatory Care Nurse (ACN) plays a vital role in our healthcare team, providing nursing care management services to assigned populations. This role requires a collaborative approach to anticipate, integrate, and coordinate patient care plans, ensuring the best possible outcomes.
Key Responsibilities- Identify at-risk patient populations and provide care management services in collaboration with physicians.
- Act as a patient/family advocate and communicate effectively with the healthcare team and community resources.
- Support the mission, values, and management of patient care services at WakeMed.
- Adhere to the ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, and WakeMed's Division of Nursing Professional Practice model.
- Diploma in Nursing or Associate's Degree in Nursing.
- Registered Nurse licensure.
- No experience required.
Monday to Thursday, Friday. No weekends or call requirements.
