About the Role

As a Hospice LPN with Heartland Hospice, you will play a vital part in delivering high-quality patient care and support to individuals and their families during times of need. Our hospice team provides compassionate, personalized assistance with daily living activities, helping patients manage the physical, emotional, and spiritual effects of serious illness or a terminal diagnosis.

About You

We are seeking a dedicated Licensed Practical Nurse (LPN) with a passion for delivering exceptional patient care. Your role will involve providing direct patient care services, evaluating patient needs, and assisting in the preparation and execution of individualized plans of care. Additionally, you will report your findings and changes in patient conditions to our interdisciplinary team and referring physicians, while maintaining accurate documentation.

Responsibilities

  • Deliver high-quality patient care services, focusing on meeting the unique needs of each patient.
  • Evaluate patient needs and develop tailored plans of care in collaboration with our interdisciplinary team.
  • Document patient progress, reporting any significant changes or concerns to our team and referring physicians.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Maintain accurate records and reports, adhering to regulatory requirements and company policies.

Requirements and Qualifications

To be considered for this position, you should possess:

  • A current LPN license to practice in the state where the position is located.
  • At least 1 year of experience in patient care, preferably in hospice, home health, ICU, geriatric, or oncology settings.
  • Demonstrated ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
  • Strong problem-solving and critical thinking skills, with a focus on providing empathetic and compassionate care.
