Job Summary

We are seeking a compassionate and skilled Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will provide direct patient care to patients in their homes, under the direction of a Registered Nurse and according to the Aide Plan of Care.

Key Responsibilities
  • Assist patients with self-administered medications, ensuring correct dosages and observing patients take their medication
  • Take accurate temperature, pulse, and blood pressure measurements, and report abnormal results to supervisors
  • Help patients maintain good personal hygiene, including bathing, grooming, skin care, and other activities
  • Assist with feeding patients, communicating basic nutrition principles, and observing and recording food and fluid intake
  • Provide necessary skills to safely assist patients with mobility, exercises, positioning, transfers, and ambulation, as per the Plan of Care and CenterWell Home Health policy
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care
  • Practice acceptable infection control principles, providing a clean, safe, and comfortable environment
Requirements
  • High school diploma or equivalent
  • Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
  • Must meet applicable state certification requirements
  • A valid driver's license, auto insurance, and reliable transportation are required
  • Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency
  • At least one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency
