Risk Assessment Provider Opportunity

We are seeking a skilled Nurse Practitioner to join our team as a Risk Assessment Provider in Kansas City, MO.

Job Summary

The successful candidate will be responsible for conducting health risk assessments on insured plan patients in their homes or designated areas. This role requires strong clinical skills, attention to detail, and excellent communication skills.

Key Responsibilities
  • Conduct health risk assessments on insured plan patients in their homes or designated areas
  • Identify active and chronic disease conditions, as well as physical, mental, and social needs
  • Obtain vital signs, measure BMI, review pharmacological therapy, and conduct physical examinations
  • Educate and counsel patients and families on identified conditions
Requirements
  • Active Nurse Practitioner state license and board certifications
  • At least 1 year of experience preferred
  • Risk Assessment coding experience/knowledge preferred
  • Valid state driver's license and automobile insurance
