Key Responsibilities:

  • Conduct initial and ongoing health assessments of participants
  • Arrange for or provide follow-up medical care as needed
  • Ensure written orders are on file for prescription medications
  • Observe participant self-administration of medication or administer if necessary
  • Instruct participants about medications, side effects, and precautions
  • Participate in formulation of medical treatment plans
  • Notify case manager, supervisor, and APNP/psychiatrists of psychiatric symptoms or behavior changes
  • Serve as liaison with other medical professionals
  • Conduct blood pressures, weights, and other medical treatments as needed
  • Document participant or collateral contacts

Requirements:

  • A two-, three-, or four-year degree from an accredited Nursing School
  • Licensure of a registered nurse
  • 1-2 years nursing experience
  • No convictions or license revocations that would fail a caregiver background check
  • Valid driver's license, automobile, and sufficient insurance
