Job Opportunity

Boro Park Center for Rehabilitation and Healthcare is seeking a highly skilled and motivated MDS Assessor to join our team.

Key Responsibilities:
  • Develop and maintain accurate and up-to-date care plans for residents
  • Monitor and audit clinical records to ensure compliance and quality
  • Collaborate with the DON to address persistent issues related to documentation
  • Protect the confidentiality of resident and facility information
Requirements:
  • Valid New York RN License
  • Excellent organizational, communication, and interpersonal skills
  • Proficiency in computer skills and software applications
  • Ability to work in a fast-paced environment and prioritize tasks effectively
