The Brookdale Hospital Medical Center is seeking a skilled MDS Assessor to join our team. As a key member of our nursing staff, you will be responsible for conducting thorough and accurate assessments of residents, ensuring compliance with state and federal regulations, and maintaining accurate records. Your expertise will be essential in coordinating assessments, preparing and transmitting MDS/PRI files, and pulling quality indicator reports. If you have a strong background in clinical nursing, excellent communication skills, and the ability to work in a fast-paced environment, we encourage you to apply.

**Responsibilities:**

* Conduct thorough and accurate assessments of residents
* Ensure compliance with state and federal regulations
* Maintain accurate records
* Coordinate assessments and prepare MDS/PRI files
* Pull quality indicator reports
* Work closely with the Quality Improvement Department
* Perform other related duties

**Qualifications:**

* Bachelor's degree in Nursing
* Minimum five years of experience in clinical nursing, including two years in performing MDS completion
* Ability to work in surroundings with degree of dust, noise, etc. without undue discomfort
* Ability to hear, talk, walk, stand, bend, and stretch
* Ability to read, write, and interpret documents
* Ability to sit for long periods of time at desk to perform job functions
* Ability to stand for long periods of time at required patient bedside and/or procedural areas
* Ability to communicate effectively with patients and/or co-workers
* May be required to operate and clean equipment
* May be required to handle reasonably heavy weights across distances
* May be required to handle/assist in lifting patients onto/out of exam beds/procedure stations
* Must be trained in and knowledgeable of all Kingsbrook/Rutland policies and procedures pertaining to hazardous waste and hazardous materials and maintain qualifications applicable to assigned work
* Licensed Registered Nurse with NYS

**Physical Requirements:**

* Normal routine involves no exposure to blood, body fluid, or tissue. Incumbents are not called upon to perform or assist in emergency care or first aid.
