About the MDS Coordinator Role:

Job Overview

We are seeking an experienced Care Coordinator to join our team at Talent Masters. As a Care Coordinator, you will play a critical role in ensuring accurate assessments and individualized care plans for our residents. This is a challenging yet rewarding opportunity to make a meaningful impact on the lives of our residents and their families.

Salary Range: $110,000 - $120,000 per year, depending on qualifications and experience.

Responsibilities:
  • Coordinate the Minimum Data Set (MDS) process to ensure accurate assessments and individualized care plans;
  • Develop, review, and adjust care plans based on resident assessments, collaborating closely with the interdisciplinary team;
  • Analyze MDS data to optimize care planning and improve quality measures;
  • Ensure timely submission of MDS data to state and federal agencies;
  • Conduct ongoing training and education for nursing and interdisciplinary staff on MDS requirements, coding, and best practices;
Requirements:
  • Active RN or LPN license in New York State;
  • Minimum 2-3 years of experience as an MDS Coordinator in a skilled nursing facility;
  • Comprehensive knowledge of MDS 3.0, care planning, and reimbursement processes;
  • Exceptional organizational skills and ability to manage multiple assessments and meet tight deadlines;
  • Strong analytical skills for data review and quality improvement initiatives;
  • Strong interpersonal and communication skills, with a collaborative approach to working with multidisciplinary teams;
