Key Responsibilities- Provide comprehensive nursing care to patients with physical disabilities and chronic illnesses
- Collaborate with healthcare providers to develop and implement individualized care plans
- Conduct thorough assessments, identify patient needs, and prioritize care
- Administer medications, perform treatments, and provide education to patients and families
- Participate in quality improvement initiatives and contribute to a positive and supportive work environment
- Current RN licensure in Illinois
- Basic Life Support (BLS) certification
- At least 3 years of experience in a clinic or ambulatory setting
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
