Key Responsibilities:
  • Collaborate with Healthcare Professionals: Work closely with physicians, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with cancer and other chronic diseases.
  • Provide Medication Therapy Management: Conduct thorough medication reviews, identify potential medication-related problems, and develop strategies to optimize patient outcomes.
  • Coordinate Care: Work with patients, families, and healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
  • Stay Up-to-Date with the Latest Developments: Stay current with the latest advances in oncology and ambulatory care, including new treatments, medications, and best practices.
  • Communicate Effectively: Develop and maintain strong relationships with patients, families, and healthcare teams through effective communication and education.
  • Contribute to a Culture of Excellence: Participate in quality improvement initiatives and contribute to a culture of excellence in patient care and service.
Requirements:
  • Education: Doctor of Pharmacy (Pharm.D.) degree or equivalent.
  • Experience: Minimum 12 months of experience in ambulatory care and oncology pharmacy practice, or completion of an ASHP-accredited PGY-1 residency program with an oncology rotation.
  • Certifications/Licenses: California pharmacist license, National Provider Identifier (NPI), and Basic Life Support (BLS) certification.
Preferred Qualifications:
  • Advanced Degree: Doctor of Pharmacy (Pharm.D.) degree.
  • Board Certification: Board certification in oncology pharmacy (BCOP).
  • PGY-2 Residency: Completion of an ASHP-accredited PGY-2 clinical pharmacy specialty residency or fellowship in oncology.
