About the Role

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Clinical Coordinator for our dialysis services. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest level of care for our patients with end-stage renal disease or chronic kidney disease.

Key Responsibilities
  • Coordinate patient care and services, including trend lab values, vital signs, and outcome management.
  • Ensure the safety, comfort, and well-being of patients by delivering high-quality care and collaborating with the clinical team.
  • Train and mentor new staff members to ensure a smooth transition and high-quality patient care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience, preferably in dialysis or a related field.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) certification is preferred.
  • Basic computer skills and proficiency in MS Word and Outlook are required.
