Hospice Social Worker Job Summary

We are seeking a compassionate and skilled Hospice Social Worker to join our team at Interim HealthCare Home Health and Hospice Careers. As a Hospice Social Worker, you will play a vital role in providing emotional and social support to patients and their families, helping them navigate the complexities of end-of-life care.

Key Responsibilities:
  • Conduct thorough psychosocial assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Provide individual and family counseling to address mental and emotional concerns, promoting a supportive and non-judgmental environment.
  • Collaborate with healthcare teams to ensure seamless care transitions and address any financial or environmental concerns.
  • Participate in IDG meetings and contribute to the development of comprehensive care plans.
  • Provide crisis intervention support and psychosocial updates to facility staff and caregivers.
Requirements:
  • Master's degree in Social Work from a Council on Social Work Education-accredited program.
  • Active licensure as a Social Worker in the state of employment.
  • Minimum one year of experience in healthcare, with hospice or homecare experience preferred.
  • Reliable transportation, valid driver's license, and car insurance.
