FeldCare Connects is a leading app-based referral network seeking a self-motivated Occupational Therapist to deliver exceptional care to adults of all ages. As an Independent Contractor, you will serve Pinole and the surrounding areas, providing premier excellence in home health care.

Job Summary

The Occupational Therapist in Home Health is responsible for assessing and evaluating patient care needs, including functionality status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment, the Occupational Therapist determines a treatment plan, performs interventions to improve patient wellbeing, and evaluates progress.

Key Responsibilities
  • Provide services defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other healthcare team members.
  • Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
  • Observe, record, and report patient response to treatment and changes to patient condition.
  • Instruct patients, families, caregivers, and healthcare team members in areas of therapy where they can participate in assisting the patient.
Qualifications
  • Ability and enthusiasm in working with all ages.
  • Occupational Therapist license and registration by the state.
  • Completion of an accredited Occupational Therapist program.
  • Bilingual is a plus.
