Job Title: Home Health Care RN

Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Care RN. As a Home Health Care RN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

Job Summary:

The Home Health Care RN is responsible for delivering and coordinating quality patient care in compliance with physician orders. This includes continuously observing and assessing patient condition and care needs, documenting all activities and assessments, and developing and implementing individualized care plans.

Responsibilities:
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and care needs.
  • Document all activities, assessments, and nursing actions.
  • Develop, implement, and update individualized care plans.
  • Take appropriate nursing action based on assessment and achieve expected outcomes.
  • Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
  • Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, and the Nurse Practice Act.
  • Provide care utilizing infection control measures that protect both staff and patients according to OSHA standards.
  • Educate patients and families regarding disease processes, self-care techniques, and prevention strategies.
Requirements:
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current, unrestricted state license as a Registered Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements:
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Must be able to sit and climb stairs.
  • Must have visual and hearing acuity.
  • Must have a strong sense of smell and touch.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
