Key Responsibilities- Develop and implement individualized care plans to meet the unique needs of patients
- Collaborate with healthcare teams to coordinate patient care and services
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes
- Associate's degree in Nursing required
- Current and active SC RN license with SCLLR
- Minimum two years RN Case Management experience in acute healthcare delivery setting preferred
- Demonstrated knowledge and experience with Case Management and outcomes required
- Experience with Epic preferred
- Knowledge in acute care level psychosocial intervention and practices required
