Job Description

As a Hospice RN Case Manager with Interim HealthCare Staffing, you will play a vital role in providing exceptional care to patients and families. Your expertise and compassion will make a significant difference in the lives of others.

Key Responsibilities:

  • Maintain a specified case management load to ensure timely and effective care delivery.
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status to determine their needs and develop a personalized plan of care.
  • Collaborate with the Interdisciplinary Group (IDG) to ensure seamless care coordination and implementation of the IDG plan of care.
  • Direct and supervise the services provided by LPN/LVN, Hospice Aide, and Homemaker to ensure high-quality care delivery.
  • Evaluate and coordinate the need for services provided to patients and families in accordance with the IDG plan of care.

Requirements:

  • Graduate of an accredited school of nursing with a valid RN license in the state of South Carolina.
  • At least 12 months of accumulated experience as a registered nurse within the last five years or evidence of current clinical knowledge.
  • One year of hospice care experience and one year of precepting or training experience preferred.
  • CPR certification and ability to provide proof of valid driver's license and auto liability insurance.
