Key Responsibilities- Perform pre-admission, admission, and continued stay reviews to ensure medical necessity and optimal resource utilization.
- Collaborate with the care team to develop and implement discharge plans that meet the individual needs of patients.
- Communicate effectively with patients, families, caregivers, and healthcare providers to ensure seamless transitions of care.
- Lead the oversight and management of functional and financial outcomes, including coordination and implementation of discharge plans.
- Adapt behavior to meet the specific needs of diverse patient populations, including respect for privacy and cultural sensitivity.
- Perform other related duties as required to support the delivery of high-quality patient care.
- Current registered nurse license in the state of practice.
- Basic Life Support (BLS) certification from the American Heart Association.
- Preferred: Certification in Case Management (CCM) or Accredited Case Manager (ACM).
- Strong interpersonal and communication skills, with the ability to present information clearly and professionally to varying levels of individuals.
- Proficiency in using computers, software, and web-based applications.
- Ability to work a flexible schedule, including evenings, weekends, and holidays.
The RN Case Manager will work in a fast-paced, dynamic environment with frequent exposure to blood, body fluids, and tissue. The incumbent will be required to wear personal protective equipment (PPE) and follow infection control protocols to minimize the risk of occupational exposure to communicable diseases.
The RN Case Manager will also be required to work in an area where patients enter, work directly with patients, and/or work with specimens that could contain diseases, which may pose an occupational risk for exposure to hazardous medications or hazardous waste.
