Job Title: Per Diem Mobile Integrated Health Nurse

Join Vashon Island Fire & Rescue as a Per Diem Mobile Integrated Health Nurse and be part of a dynamic treatment and outreach team that coordinates and delivers health services in the Vashon community setting.

Job Summary:

The Per Diem Mobile Integrated Health Nurse is responsible for responding to scheduled patients at their home and connecting patients to acute care services on an as-needed basis. This role requires a strong understanding of medical terminology, critical thinking, and clinical reasoning skills.

Key Responsibilities:
  • Respond to scheduled appointments with patients who have low-acuity medical, behavioral, and social needs.
  • Deliver appropriate acute care services, including moving, lifting, or transferring patients as needed.
  • Identify high-risk clients with complex needs and connect them to whole-person care services.
  • Work collaboratively with a multidisciplinary team, including Firefighter/Emergency Medical Technicians and the Mobile Integrated Health Unit Program Coordinator.
  • Pilot test new workflows and digital tools to improve tracking and communication between health and social service agencies.
  • Support Public Relations and/or Public Education events as needed.
  • Attend program-related conferences/training to stay up-to-date on industry developments.
Requirements:
  • Unencumbered RN license in the State of Washington.
  • Certified in CPR (Health Care Provider).
  • Minimum of 5 years' experience as an RN, preferably in a community/outpatient setting providing care management/coordination.
  • BSN/BA in nursing or related field.
  • Experience working with elderly or Public Health background preferred.
Preferred Qualifications:
  • Spanish or other second language ability.
  • 5+ years of experience in clinical healthcare setting, home health, chronic care, or other related acute care healthcare delivery setting.
  • Experience working with underserved or vulnerable populations.

Pre-Employment Requirements:

Final candidate will need to successfully pass a background screening that includes driving records, credit check, and drug test, and a psychological interview process. The final candidate must pass a pre-employment medical physical.
