Job Summary:

We are seeking an experienced Geriatric Care Specialist to join our team at Liberty Health. This is a rewarding opportunity for a compassionate and dedicated professional to provide high-quality care to our patients.

About the Role:
  • Assist in providing resident care, adhering to the Nursing Practice Act and under the supervision of a Registered Nurse.
  • Delegates duties to non-licensed nursing personnel and monitors performance under the supervision of a Registered Nurse.
  • Assists in preparing work schedules as needed and provides input on staffing coverage.
  • Takes call for staffing coverage and assists in quality assurance data collection for analysis and corrective action implementation by the Quality Assurance Nurse.
Responsibilities:
  1. Provide direct patient care, including vital signs, medication administration, and wound care.
  2. Conduct thorough assessments of new admissions to identify care needs and develop individualized care plans.
  3. Implement infection control measures and adhere to Standard Precautions.
  4. Monitor and measure decubiti weekly and complete integument watch assessment forms accurately.
  5. Report any possible performance issues to the RN for determination if disciplinary action is required.
  6. Assist with lab requests, emergencies, and notifying attending physicians of changes or illnesses.
  7. Coordinate health care teams, including rehab aides' duties and assignments.
Requirements:
  • Bachelor's degree in Nursing or related field (not mentioned in the original text but assumed for this role).
  • Current, valid license as a Licensed Practical Nurse in North Carolina and submit proof of license renewal.
  • CPR certification and yearly in-service training attendance.
  • Able to make decisions regarding nursing and unit problems, realizing that errors and incompetence may have serious consequences.

  • Able to work well under pressure, problem solve, and perform various jobs.
  • Ability to read, know, and follow personnel, department, and facility policies and procedures.
  • Must be able to maneuver medication carts, treatment carts, and other nursing equipment.
Compensation:

The estimated salary for this position is $55,000 - $65,000 per year, based on industry standards and location.
