Job Overview

We are seeking a skilled and compassionate Nurse Specialist to join our geriatric care team at Liberty Healthcare and Rehabilitation Services.

Job Summary:
  • Assist in delivering exceptional patient care by working closely with nursing staff on designated units during assigned shifts.
  • Provide support for staffing coverage as needed, including responding to call duties.
  • Collaborate with Registered Nurses to ensure high-quality resident care in accordance with the Nursing Practice Act and relevant regulations.
  • Delegate tasks to non-licensed nursing personnel, monitoring their performance under the supervision of Registered Nurses and assist in preparing work schedules as required.
  • Contribute to Quality Assurance data collection and analysis, facilitating corrective actions implemented by the Quality Assurance Nurse.
  • Support infection control initiatives for the unit, adhering to regulations regarding cleanliness, infection control, and Standard Precautions.
Responsibilities:
  1. Participate in preparing initial nursing assessments for new admissions, ensuring accurate documentation.
  2. Complete assigned resident assessments, collaborating with MDS Nurses or Directors of Nursing as necessary.
  3. Attend scheduled nurse department meetings, fostering open communication and collaboration.
  4. Monitor and measure decubiti weekly, accurately completing integument watch assessment forms and coordinating with other departments to address unit needs and patient requirements.
  5. Report potential performance issues among employees to RNs for determination of disciplinary action, if necessary.
  6. Support lab request procedures, using proper techniques and re-posting lab results as ordered on the unit.
  7. Respond to emergencies arising during duty hours, performing CPR as needed, and assisting with emergencies on other units when required.
  8. Notify attending physicians of patient changes, illnesses, or deaths within the unit, implementing ordered changes in care as instructed.
  9. Provide general support, including rounds with doctors, checking MD orders, updating records, and verifying completeness of nursing summaries.
  10. Be available for hall coverage as needed and contribute to coordinating the healthcare team, including rehab aide duties and assignments.
Requirements:
  • Must possess a high school diploma or equivalent.
  • Hold a current, valid Licensed Practical Nurse (LPN) license in North Carolina, requiring proof of renewal.
  • Provide quality nursing care, adhere to doctor's orders, and demonstrate genuine interest in geriatric nursing.
  • Maintain CPR certification annually and attend in-service training and staff meetings as required.
  • Demonstrate ability to make decisions regarding nursing and unit problems, considering potential consequences for patients and staff.
  • Possess strong problem-solving skills, flexibility, and the capacity to perform multiple tasks effectively.
  • Familiarize yourself with facility policies, local, state, and federal regulations, and adhere to established guidelines.
