Position Summary
The Physical Therapist operates under the guidance of the Facility Manager, focusing on delivering exceptional outpatient rehabilitation services through a collaborative care model with our medical team to enhance patient performance and quality of life.
Core Responsibilities
- Deliver outstanding patient care.
- Interpret physician orders, conduct initial evaluations, and develop treatment plans with functional objectives.
- Regularly update treatment goals as necessary throughout the care process.
- Document patient progress in compliance with organizational guidelines.
- Determine appropriate discharge timelines and follow-up care.
- Supervise Rehabilitation Technicians and support staff as required.
- Maintain compliance with documentation and billing policies.
- Stay current with continuing education requirements to maintain an active state license.
- Foster positive relationships with physician teams.
- Engage actively in the medical team, including office visits and communication with case managers.
- Participate in organizational programs such as mentoring and community outreach.
Qualifications
- Graduate from an accredited Physical Therapy Program.
- Licensed Physical Therapist or eligible to take the licensing exam in Illinois.
- Ability to thrive in a fast-paced environment.
- Strong written and verbal communication skills.
- Capability to lift over 25 pounds.
- Understanding of proper body mechanics for lifting and moving patients.
- Ability to stand or sit for extended periods.
- Proficient in using a computer for documentation and communication.
