Director of Social Services and Patient Care

Oceanside Center - Hampton, NH is seeking a highly skilled and compassionate Director of Social Services and Patient Care to join our team. As a key member of our interdisciplinary team, you will play a vital role in creating a supportive and inclusive environment that promotes the well-being and dignity of our patients.

Key Responsibilities:
  • Develop and implement comprehensive social services programs that address the unique needs of our patients.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Supervise and mentor social services staff to ensure high-quality care and services.
  • Conduct thorough psychosocial assessments to identify patients' social, emotional, and psychological needs.
  • Provide education and support to patients and families on their rights, responsibilities, and available community resources.
Requirements:
  • Bachelor's degree in social work or human services required.
  • Must possess any certifications/licensures as required by state of employment to practice in long-term care.
