Job Title: Hospice Care Specialist

At Tufts Medicine, we are seeking a compassionate and skilled Hospice Care Specialist to join our team. As a Hospice Care Specialist, you will provide personalized care and support to patients and their families, ensuring a high-quality end-of-life experience.

Key Responsibilities:

  • Provide patient care and support in a variety of settings, including private homes, assisted livings, and skilled nursing facilities.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals to ensure seamless care coordination.
  • Provide emotional support and education to patients and their families.

Requirements:

  • High school degree or equivalent.
  • Certified Home Health Aide or Licensed Nurse Aide.
  • Valid driver's license.
