Nurse Practitioner/Physician Assistant

Matrix Medical Network is seeking a skilled Nurse Practitioner/Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing their current health, medical history, medication adherence, social environment, and other risks.
As a Home Risk Assessment Specialist, you will work collaboratively with physicians, case managers, social workers, family members, and caregivers to provide personalized care and support. You will also have the opportunity to work with a nationwide network of advanced practice providers, building and maintaining relationships to improve patient outcomes.

Responsibilities
As a Home Risk Assessment Specialist, your responsibilities will include:
- Conducting comprehensive health assessments in patients' homes, reviewing their current health, medical history, medication adherence, social environment, and other risks.
- Working collaboratively with physicians, case managers, social workers, family members, and caregivers to provide personalized care and support.
- Collaborating with Primary Care Physicians (PCPs) on patient education and follow-up.
- Building and maintaining relationships with a nationwide network of advanced practice providers.
Qualifications
To be considered for this role, you must have:
- A Master's Degree in Nursing or a related field, or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP/PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
- A minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- A positive and outgoing personality, with a strong ability to work collaboratively as part of a team.
