Nurse Practitioner

**About the Role**
At Tech Tammina, we are seeking a highly skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in delivering high-quality patient care and working collaboratively with our interdisciplinary team to achieve exceptional outcomes.
**Responsibilities**
• Provide comprehensive nursing care to patients, including assessment, planning, implementation, and evaluation of care plans.
• Conduct thorough assessments of patients, including gathering medical history, performing physical examinations, and collecting data from caregivers and other healthcare professionals.
• Develop and implement individualized care plans that address the unique needs and goals of each patient.
• Teach patients and families about health care regimens, disease management, and wellness strategies.
• Administer medications, tests, and treatments as ordered by healthcare providers, ensuring safe and effective delivery.
• Manage medication refills and coordinate with healthcare providers to ensure timely and accurate medication management.
• Collaborate with the interdisciplinary team to provide comprehensive care and support to patients and their families.
• Complete biannual and status change nursing assessments for assigned patients, documenting problems, goals, and interventions in the patient's care plan.
• Utilize assessment tools, such as the PACE Nurse Assessment and Medication Management, to collect data and inform care decisions.
**Requirements**
• Baccalaureate degree in nursing preferred; current Massachusetts license in nursing required.
• Current BLS certification required.
• Minimum of 2 years' experience in ambulatory, home health, hospital, or sub-acute settings preferred; equivalent training or related experience considered.
• A minimum of one year's experience working with a frail or elderly population.
• Prior experience in an interdisciplinary team setting a plus.
