Wound Care Nurse

We are seeking a skilled Wound Care RN to join our team at Providence Home and Community Care. As a Wound Care RN, you will be responsible for providing high-quality care to patients in their homes, focusing on wound management and case management.
Responsibilities- Provide wound care services to patients in their homes, including wound assessment, dressing changes, and education on wound care and management.
- Develop and implement individualized care plans for patients, taking into account their unique needs and goals.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care for patients.
- Conduct home visits to assess patients' needs and provide education on wound care and management.
- Document patient information and progress in a timely and accurate manner.
- Graduate of an accredited school of nursing.
- Current Washington State Registered Nurse License.
- Basic Life Support (BLS) certification.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Bachelor's Degree in Nursing.
- 2 years of experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility.
- 1 year of recent, hands-on technical clinical experience in a Medicare-certified Homecare or Hospice agency/facility doing intermittent visits and case management.
- Recent hands-on experience in wound care, venipuncture, ostomy, catheters, and feeding tubes.
- Rehab nursing experience.
- Experience working with an EMR system.
- Experience with office software programs including Outlook, Word, and/or Excel.
