This RN Nurse Care Coordinator role will be an essential element of an Integrated Care Model by relaying pertinent information about member needs and advocating for the best possible care available. We ensure members have the right services to meet their needs.
Our team is made up of RNs, LVNs, and CACs dedicated to helping members achieve their healthcare goals. We work closely with nursing facilities in which our members reside.
This is a field-based position with a home-based office. We require reliable transportation and the ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities, or providers' offices.
Primary Responsibilities:
- Assess, Plan, and Implement Care Strategies that are individualized by member and directed toward the most appropriate, least restrictive level of care
- Identify and Initiate Referrals for member transition to community requests and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services
- Manage the Care Plan throughout the continuum of care as a single point of contact
- Communicate with Stakeholders for required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for Members and Families as needed to ensure member needs and choices are fully represented and supported by the healthcare team

Required Qualifications:
- Current, unrestricted RN license in the state of Texas
- 2+ years of experience working within the community health setting or in a healthcare role
- Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications
- Reliable transportation and the ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
