Supervision: Reports to Lead Clinician
Role Summary
The Clinician for NYAP's La Jornada (The Journey) Program is dedicated to making a significant impact in the lives of minors who have arrived in the U.S. without parental or legal guardian support. This role involves close collaboration with minors, foster families, sponsor families, and community partners, delivering therapeutic services, advocacy, and support aimed at fostering safe and successful community integration.
Key Responsibilities
- Deliver all services in alignment with the National Youth Advocate Program's mission and values.
- Manage intake, placement, and discharge processes, including conducting and updating assessments and service plans, and ensuring timely documentation in the electronic health record (EHR) system.
- Document progress notes and related materials, ensuring compliance with EHR and governmental database requirements.
- Conduct regular risk assessments and develop safety and crisis management plans for minors.
- Engage with foster and sponsor families to review and ensure the accuracy of all documentation.
- Provide individual, group, and family mental health services, meeting established outcome benchmarks.
- Travel to various locations to deliver in-home services, adhering to organizational and regulatory standards.
- Coordinate community services for minors and families, including medical, educational, and psychological support.
- Screen for potential human trafficking issues and provide crisis intervention as needed.
- Assist in placement efforts consistent with the minors' case plans.
- Perform additional duties as assigned.
- Master's degree in social work with clinical experience; must be licensed or eligible for licensure; or a bachelor's degree with 5 years of clinical experience and must be licensed or eligible for licensure (if applicable).
- Flexibility to work non-traditional hours, including potential on-call responsibilities.
- Ability to organize and escort youth to various travel destinations, including potential out-of-state trips.
- Valid driver's license and reliable vehicle required.
- Capacity to adapt and respond to the needs of youth, families, and community partners.
- Professionalism and objectivity in all job functions.
- Bilingual proficiency in English and Spanish is essential for effective communication with clients.
- Proficient in using technology including computers, smartphones, and relevant software applications.
