Key Responsibilities


The Hospice Aide, under the guidance and continuous supervision of a registered nurse, is responsible for delivering personal direct patient care services as outlined in the Plan of Care on a daily basis.


- Communicates with the Executive Director or designated personnel after each patient visit regarding the patient's condition or environment, adhering to the Care Management process as necessary.


- Assists with essential Activities of Daily Living (ADLs) during each patient visit, including mobility, transfers, grooming, bathing, dressing, eating, and toileting.


- Performs incidental household tasks that are crucial to the patient's health care at home, ensuring services are provided to prevent or delay institutionalization.


- Engages in the Quality Assessment and Performance Improvement (QAPI) process by identifying issues and reporting to the administrator as needed.


Must possess reliable transportation.

Qualifications


License Requirements:
- Successful completion of a competency evaluation prior to providing patient care.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to reliable transportation.


Additional State-Specific Requirements:
NJ:
- Certification mandated by the New Jersey Board of Nursing.

Skills Required


- Mature and capable of effectively managing the demands of a hospice aide role.
- Ability to read, write, and follow directions promptly and accurately.
