Position Overview
We are seeking a dedicated LPN Care Coordination Specialist to join our team. This role will report directly to the Director of Advanced Illness Management (AIM) and will be responsible for overseeing daily operational activities and clinical functions within the AIM and Palliative Care department.

Key Responsibilities
1. Coordinate and manage adherence to visit schedules and clinical activities.
2. Handle incoming communications from partners and patients.
3. Ensure compliance with visit frequency, including scheduling PRN visits and follow-ups post-hospitalization.
4. Facilitate timely discharge home through effective communication with partners and scheduling of post-hospitalization visits.
5. Order necessary labs, durable medical equipment (DME), and imaging services, and coordinate results with Nurse Practitioners.
6. Maintain accurate medical records and documentation in electronic medical record systems.
7. Assist in identifying patients eligible for hospice services and coordinate referrals.

Qualifications
- Bachelor's degree in Health Care Administration, Nursing, or a related field.
- A minimum of three years of experience in healthcare delivery or a related business environment.
- Preferred experience in a Part B billing setting.
- Valid LPN License required.
- Proficiency in computer applications and electronic medical records.
