Job Summary

As a Sweet Dreams Certified Nursing Assistant, you will partner with our Director of Patient Care Services and Interdisciplinary Teams to further that mission. These passionate teams will depend on you to care for and support our patients during their hospice journey.

Responsibilities
  • Provide quality one-on-one time with patients and their families to support everyday needs
  • Care for hospice patients by providing personal care services, including bathing, grooming, and oral hygiene
  • Assist patients with ambulation and exercise
  • Record important health information, including temperature, pulse, respiration, and blood pressure
  • Assess and report patient condition and any significant changes to the Case Manager
  • Provide patient and family with positive communication techniques, within the realm of ethical and respectful care, including confidentiality
  • Help patients dress and undress
  • Perform additional duties as assigned
Requirements
  • An enthusiasm for providing the best possible care to patients and their families
  • A CNA or HHA license in good standing
  • Must be able to work in the evenings, generally from 3:00 p.m. to 9:00 p.m.
  • Must be flexible in work hours to ensure patients receive care as provided in the plan of care
  • 1 year of experience in hospice or home health preferred
  • A valid driver's license with an automobile in good working order that is properly registered and insured
  • Current CPR certification
  • Excellent observation, verbal, and written communication skills
